I. Learning objectives
By the end of the lesson, students will be able to:
know the steps to do a survey on student’s exercise habits;
know the rules for designing a good questionnaire;
improve social skills by approaching the target respondents properly and ask them to fill in the questionnaire;
know how to analyse the statistics from the questionnaire;
know how to write a report to present the results of the survey and make suggestions on improving students’ exercise habits;
cooperate effectively with group members, be clear about their assignments in the group work and complete the assignments.

IV. Teaching procedures
Step 1 Lead-in
1. T asks Ss to review the benefits of exercise.
2. T shares his or her exercise habits and invites some students to share their exercise habits.
3. T points out the task of this class: help our PE teacher do a survey on students’ exercise habits.
【設計意圖:通過復習前面所學的鍛煉益處,自然過渡到我們應該加強鍛煉的話題。老師分享自己的鍛煉習慣,拉近和學生的距離并激起學生的分享欲望。由學生們不同的鍛煉習慣引出本課任務:幫助體育老師做調查,以了解全校學生的運動習慣。】
T shows Ss the steps to do a survey.
Step 2 Planning
T asks Ss to talk about what to ask on the questionnaire about the exercise habits of the students in the school.
Students can use the ideas in the textbook:
How often do you exercise
How long do you exercise
What type of exercise do you do
Where do you exercise
Students can think of more ideas:
Who do you usually exercise with
When do you usually exercise in a day
Do you drink throughout exercise
Do you warm up before exercise
Do you stretch after exercise
Do you wear proper clothes and equipment

Step 3 Preparing
Rules for designing a questionnaire
T asks Ss to compare each question in the two samples and discuss which one is better.
Students summarize the rules for designing a questionnaire:
Give specific references.
Ask about only one piece of information at a time.
Give enough options.
Use short and simple sentences.
Make sure that the respondents have the necessary knowledge to answer the question.
Use simple and short questions.
Use precise language.

2. Design a questionnaire
T asks Ss to get divided into groups and design a questionnaire together.
T: Now in groups, design your own questionnaire. Don’t forget the six rules of questionnaire design. You can refer to the questions you’ve talked about before.

Step 4 Producing
T offers Ss some useful expressions for approaching the respondents and carrying out the survey.

3. T asks Ss to write a report.
(1) T asks Ss to read the sample report and find out its structure.
Yesterday, we carried out a survey on students’ exercise habits in our school. The results are as follows. (Specify the time, place and respondents of the survey)
From the diagram, we can see that 40% of students exercise less than once a week or never exercise. Experts recommend exercising at least three times a week, but we find only 28% of students manage that. (Summarize the results of the survey)
In my opinion, schools and teachers should stress the great benefits of exercise and organize more activities or competitions to develop students’ interest in exercise. Besides, PE lessons should be made full use of to ensure the required time spent on exercise. (Make suggestions)
